background: conventional doppler measurements, including mitral inflow and pulmonary venous flow, are used to estimate left ventricular end diastolic pressure (lvedp). however, these parameters have limitations in predicting lvedp among patients with mitral regurgitation. this study sought to establish whether the correlation between measurements derived from tissue doppler echocardiography and lvedp remains valid in the setting of severe mitral regurgitation. methods : thirty patients (mean age: 57.37 ± 13.29 years) with severe mitral regurgitation and a mean left ventricular ejection fraction (ef) of 46.0 ± 14.95 were enrolled; 16 (53.4%) patients were defined to have ef < 50% and 14 (46.6%) patients had ef ≥ 50%. doppler signals from the mitral inflow, pulmonary venous flow, and doppler tissue imaging indices were obtained, and lvedp was measured invasively through cardiac catheterization. results : the majority of the standard doppler and doppler tissue imaging indices were not significantly correlated with lvedp in the univariate analysis. in the multiple linear regression, however, early (e) transmitral velocity to annular e' (e/e') ratio (ß = 1.09, p value < 0.01), e wave velocity to propagation velocity (e/vp) ratio (ß = 7.87, p value < 0.01), and isovolumic relaxation time (ß = 0.21, p value = 0.01) were shown as independent predictors of lvedp (r 2 = 91.7%). conclusion: the ratio of   e/vp and e/e' ratio and also the isovolumic relaxation time could be applied properly to estimate lvedp in mitral regurgitation patients even in the setting of severe mitral regurgitation.

Doppler Tissue Imaging: A Non-Invasive Technique for Estimation of Left Ventricular End Diastolic Pressure in Severe Mitral Regurgitation

BACKGROUND Conventional Doppler measurements, including mitral inflow and pulmonary venous flow, are used to estimate left ventricular end diastolic pressure (LVEDP). However, these parameters have limitations in predicting LVEDP among patients with mitral regurgitation. This study sought to establish whether the correlation between measurements derived from tissue Doppler echocardiography and ...

Non-invasive estimation of human left ventricular end-diastolic pressure.

Sato et al. (Electronic Letters 32, 949-950, 1996) reported that one can obtain a non-invasive estimate of left ventricular (LV) pressure at around end-diastole in an isolated canine preparation. In this study we examined whether this method can be applied to humans. Using the method proposed by Kanai et al. (IEEE. Trans. UFFC, 43, 791-810,1996), we detected small amplitude LV vibration from an...

assessment of subclinical left ventricular dysfunction in patients with chronic mitral regurgitation using torsional parameters described by tissue doppler imaging

background: left ventricular (lv) twist is due to oppositely directed apical and basal rotation and has been proposed as a sensitive marker of lv function. we sought to assess the impact of chronic pure mitral regurgitation (mr) on the torsional mechanics of the left human ventricle using tissue doppler imaging. methods: nineteen severe mr patients with a normal lv ejection fraction and 16 non-...

Doppler echocardiographic estimation of left ventricular end-diastolic pressure after MI in rats.

The spectral Doppler mitral flow pattern, alone or combined with tissue Doppler mitral annulus velocity, can be used to predict left ventricular (LV) filling pressure in humans, whereas invasive hemodynamic measurements are still required in the rat. This study was undertaken to assess whether LV end-diastolic pressure (LVEDP) can be estimated using Doppler echocardiography in the rat after myo...

Mitral flow derived Doppler indices of left ventricular diastolic function.

AIMS Left ventricular diastolic dysfunction has been proposed as the basis of heart failure in patients with normal left ventricular systolic function. Doppler indices of mitral inflow have been widely used to diagnose this condition and have been shown to correlate well with increased left atrial pressure in patients with cardiovascular disease. We wanted to establish age-specific criteria for...

Mechanics of left ventricular contraction in chronic severe mitral regurgitation.

The mechanics of left ventricular contraction were studied during diagnostic cardiac catheterization using high-speed cineangiography in 11 patients with severe chronic mitral regurgitation. Compared with a group of previously studied normal subjects, most of the patients with mitral regurgitation demonstrated a reduced velocity of shortening (Vf) during ejection at maximum wall stress, average...
